Prior to Appointment

Office staff will verify your insurance.

Check with your health insurance carrier for final determination regarding specific covered services since they are subject to change.
Day of Service

Applicable co-payments, co-insurance and deductible amounts will be collected.
Please Note : Patients must notify staff if there are any changes to insurance coverage since their last visit, at least 72 hours before next appointment. Failure to due so will result in the patient rescheduling their appointment, until new insurance is verified.
